2023 VOWSA Practice Swim Assumption of Risk

To be signed by anyone under the age of 19

Assumption of Risks:

I am aware that my participation in VOWSA practice swims in open water involves many risks, dangers and hazards, which could result in damage, loss, serious physical injury, or death to me. Some of these risks, dangers and hazards include, but are not limited to, physical traumatic injury from boats or other water vessels, drowning, hypothermia, cardiovascular problems, permanent disability, overexertion, dehydration, fatigue, lack of fitness or conditioning, other injuries, bacterial infections, rashes and the transmission of communicable diseases, including viruses of all kinds, bacteria, parasites or other organisms, or any mutation thereof.

I am also aware that my participation in VOWSA practice swims may put me at an elevated risk of contracting or being exposed to viruses, contagious diseases or other illnesses that may be present in the general population and/or in public spaces and that I nevertheless choose to participate in the VOWSA practice swims and fully assume the risk of doing so.

To the best of my knowledge, my physical condition and fitness are adequate for me to compete in VOWSA practice swims for which I have submitted request for inclusion, and I am unaware of any reason, physical or otherwise, why I should not participate. I agree to comply with all rules, regulations and event instructions of the VOWSA practice swims and I consent to receive any and all medical treatment which organizers of the VOWSA practice swims consider advisable in the event of illness or injuries suffered by me during any VOWSA practice swim.
